Blameless Post-Mortem
A blameless post-mortem is an incident review methodology that deliberately focuses on understanding systemic causes, process failures, and improvement opportunities rather than assigning personal blame to individuals involved in an AI system failure or incident.
What this means in practice
By creating a psychologically safe environment for honest reporting, blameless post-mortems produce more accurate analysis and more effective preventive measures than blame-oriented reviews, which incentivize hiding mistakes and deflecting responsibility. For organizations operating AI in production, this approach is essential because AI failures are almost always systemic rather than attributable to a single person's error. In COMPEL, blameless post-mortems are part of the operational resilience framework in Module 2.4, Article 12, and connect to the organizational learning practices in the Learn stage.
Why it matters

Blameless post-mortems are part of the operational resilience framework established during the Model stage within the Process pillar. During Produce, post-mortem protocols are activated when AI incidents occur. The findings feed directly into the Learn stage's continuous improvement processes, where systemic causes are addressed rather than individual blame assigned. The Evaluate stage tracks whether post-mortem recommendations are being implemented and reducing incident recurrence.
